If you received a Plan Notice, you have been identified as a Preliminary Claimant in the Cetera Advisors Fair Fund and may be entitled to receive a Distribution Payment from the Cetera Advisors Fair Fund. In order to receive a Distribution Payment, you must complete a Certification Form, confirming the Excess Fee Amounts that are pre-populated within your Certification Form or disputing the Excess Fee Amount(s) no later than April 22, 2024.
If you fail to submit a Certification Form (submitted online or postmarked if mailed) by April 22, 2024, you will not be eligible for a distribution under the Plan.
Remember: All Certification Forms must be submitted online or postmarked no later than April 22, 2024.

The Unique ID is a 10-character alphanumeric value and the PIN is a 4-character numeric value, both of which can be found on the Certification Form that you received.
